one underappreciated aspect of free-market analysis is that the core of Austrian theory defines price as an information mechanism; when price is pervasively distorted - the first and highest-priority goal of anybody seeking to capture power - that informational signal disappears, which means that we can no longer reason effectively about allocation of inputs or outputs

we see this right now: how much health care is “just right”? who should get it? how much “should” it cost? how much “should” somebody get paid? fuck, who knows? screw everybody as hard as you possibly can, and if they’re not punching out, fuck’em harder! (and if they do punch out, fuck’em TWICE as hard! they’ll come back!)

we can’t undo the existing grants of privilege underlying the present fucked incentive-structure without causing the system to suffer a (literal) fatal collapse; unless you’re willing to see preventable deaths by the millions due to the current provisioning system “waiting out the bad market conditions” by withholding care and treatments (something which is DEEPLY UNETHICAL but is a guaranteed consequence), there are a lot of people that will need to spend some quality time subjected to Hitlerian Healthcare Command Economics, at least until the ecosystem has been restored somewhat to a natural state where it can be liberalized again

patent law gets updated: you want to claim an “innovation”, great, but then your FULL schematics and parts lists for that device become open-source when the patent covering it is up

software, same thing: open standards and protocols, no more “custom software” for this shit that costs bazillions of dollars on the subscription model (“pay $X million per year! such value, goyim!”)

pharma gets a taste of this too: they’re riddled with “Hollywood Accounting” where they justify $10,000/pill charges “because we have to pay for the ones that didn’t work goy anon, don’t you want to support the FREE MARKET and PROGRESS?” (lmao no patents are by definition anti-“free market”, there is a tradeoff we make for prosocial outcomes and we’re not getting those outcomes so we are altering the deal, pray we do not alter it further)

if you want to subsidize your losers, that’s your business, and you can have market exclusivity for the patent period, but the price is absolutely invariant: you make X number of pills of Miraculosine, great, then you charge $Y per pill to all comers and that’s the end of it, and that covers “everybody everywhere in the world”, we’re not subsidizing niggers any more by pumping money out of White people who need health care and have been too nice to gas all the fucking kikes swindling them (so far)

also, the “tweak the formulation a little bit and get a ‘use patent’ for it, then jack up the price again” monstrosity gets gassed, and the “do a halfass study about a drug that’s been available for 200 years, get exclusivity, then jack up prices to the moon” gets some gas too, we’re done with that heinous bullshit

@Humpleupagus @fknretardlol @PNS @captainslow the perennial observation is that no provider in this system has any incentive to change anything, which is why it doesn’t change, and since they are effective gatekeepers, all they have to do is say “lmao no” and refuse to participate, at which point people needing medical care will be squeezed until they give up and return to the old way of doing things

the hard reset looks something like this:

• every licensed medical professional in the country gets drafted into the Public Health Service, at rank equivalent to O-1 to O-3 (depending on experience), and subject to a cadre of disciplined senior doctors who are fully on-board with the program and who have no sense of humor (think Chairman Goodchild as an O-9) - critically, they become subject to the UCMJ across the board
• all hospitals, clinics, and laboratories become eminent-domained into the PHS too
• pay for the PHS personnel is commensurate with their new (quasi-military) rank
• all medical debts are forgiven
• all medical-school student loans are forgiven
• insurance is outlawed
• refusal of orders is punished as, well, refusal of orders
• failure or refusal to perform to the standards of the profession is punished as malingering and/or dereliction of duty
• attempts to flee are punished as desertion
• all incompetent niggers and jeets are bounced instantly (they become janitors or get deported)
• each credentialed professional that remains is responsible for precepting no fewer than ten intelligent White students from the local high school, all the way through residency, at which points those new professionals will be commissioned in the PHS at roughly O-2
• failure to precept effectively is also punished as refusal of orders and/or malingering
• add AI diagnosis pre-screening (publicly available; people are using Dr. Google anyway, may as well formalize it)
• remove pharma gatekeeping function (prescriptions are over, you buy the shit at the supermarket; scheduled substances may have a hoop to jump through, though)
• remove test gatekeeping for most things (all standard blood work is available on demand, most imaging modality is available on-demand, things that involve nontrivial levels of danger are available with a presumption that you know what you’re doing and aren’t too interested in dying on the table, but there can be some guardrails around things like that; if it requires a surgeon, anesthesia, large doses of radiation, or dangerous chemicals, at least a mandatory explanation of risk is necessary; this runs the risk of killing off some Munchausen’s cases or hypochondriacs, but that’s a different conversation and the boost to autonomy to my mind outweighs that risk)

this lasts for ten years or so, or “long enough for the wave of bankruptcies to propagate through the system and resolve”, and long enough for their expectations to reset - the average doc will live a pretty decent middle-class lifestyle and will deal with a ton less bullshit, but does not have an automatic ticket to the UMC, and certainly isn’t going to be billing $2000/hr for his time, and the average nurse is going to be able to afford a decent apartment in a reasonable area but isn’t going to be minting cash

after that ten-ish years is up, dismantle it: sell off the clinics, begin “retiring” the professionals onto the “inactive” list, with the understanding that if their hands start to clasp and their nose starts growing and hooking downward again, they’ll be re-inducted and will live the rest of their life doing 6am uniform inspections 6 days per week until they age out of the system; inform people with the bright idea of recreating the insurance nightmare that they are not going to be doing that and that their idea is going to get them stood up against a wall after enjoying a brief cigarette in the dawn sunshine

the only way to cure something this systemically fucked and re-align it to something where “market dynamics” can operate effectively is when the mechanisms of capture that have been cemented in place are forcibly broken beyond reconstitution, and that requires non-consensual participation of some currently-benefiting participants (rape)

the problem is that that only works when, as noted, everybody in the structure (including those running it) is cooperating, which hasn’t been true for a real long time; for a supermajority of organizations, and for a very long time now, the game has been “who can screw the other guy first/hardest”

I’ve seen wealthy high-end professionals with assistants who generate half their income balk at paying more than $25/hr post-COVID, and attempting to explain to them that “uh this person is the reason you’re able to even be in business at this location, open your checkbook” falls on deaf ears; this is the fundamental problem and nothing gets fixed until that mindset goes away, the big boss is not a priori entitled to the infinite money glitch in a universe where his employees can’t even afford to buy a fucking house and that problem is going to be solved one way or the other, the only actual choice being offered to such men is whether or not they’re going to cough up the cash voluntarily or whether the entire system gets burned down with them in it (and people are becoming increasingly okay with that idea)

the “temporarily embarrassed millionaire” American delusion is wearing very thin and reality is beginning to seep in around the cracks
