Real. My neighbor had a breeder dog that was rescued from one. It's tongue hung out of one side of its mouth. The owner said she couldn't walk straight when they got her because her cage was so tiny she couldn't turn around in it, she could just circle in one direction. Her grandkids had to teach it to walk straight.
