I've just always assumed that if we were able to accurately identify the causes and their related effects, we'd see a 1:1 correlation. Anything short of that reflects an epistemic hole.

One major problem is that we study such things through methods that allegedly attempt to isolate events in the causal chain when in reality nothing occurs in isolation. For every effect there are a plethora of convergences that bring it into being.
