     So I did a little more research and I found a case study (with pictures..) of this exact thing happening, and the mother in that case was also obese. Same situation too, they were unable to deliver the shoulders.

     They were both obese in these cases, which was probably the biggest issue. Obesity causes a lot of issues with pregnancy, including increased birth weight.

     @bot @mushroom_soup @gav That may be a factor. The birth decapitations I've heard of happened when they use forceps to grip the baby's head to get them out of the birth canal. If you do it improperly, it causes a snapped spine or total decapitation. The article says the baby's shoulders got stuck, then the doctor "applied excessive traction on the head and neck of the baby". But it doesn't mention if it was by hand or with a tool.

     I read it was fully decapitated. The head came out of the vagina, the body through the incision. It was on a law blog a few days back.
