The birth decapitations I've heard of happened when they use forceps to grip the baby's head to get them out of the birth canal. If you do it improperly, it causes a snapped spine or total decapitation. The article says the baby's shoulders got stuck, then the doctor "applied excessive traction on the head and neck of the baby". But it doesn't mention if it was by hand or with a tool.
