The Blackhawk replied that they had the aircraft in sight.

THEY WERE LOOKING AT THE WRONG AIRCRAFT. 😢

That's it. That's the whole shebang. There's nobody to blame except the Blackhawk pilot who wasn't able to see the other plane coming in for the landing from his angle.
