The system was designed with multi-redundancies, and the elite started swapping foreigners in, and thought "oh man, we can just replace all of our people with easy to control foreigners" .. but didn't realize that they were blowing through the safeties.

It's like what I've said with QA before. Your QA system should not be saving your bacon. If a lot of things are failing QA, that is an alarm bell that you need to take a hammer to your whole process.
