The thing that doesn't happen is all of the records getting lost. That's because mishandling medical records means doctors and hospital adminstrators and IT people can go to prison. Because once they get caught (they always get caught) they catch penalties that wipe out whatever profit margins they eked out from the fraud. Hospitals aren't like banksters, they can't factor in massive fines as the cost of doing business. And if a US hospital loses the ability to charge Medicare/Medicaid that is usually a death sentence. Not only will they hit you with a five or six digit fine that you have to pay, in full (no negotiation) but every time you apply or reapply for hospital privileges they specifically ask you "have you lost the ability to take Medicare/Medicaid at any point in your career?". It's like a dishonorable discharge for medical professionals. -

I think the case I'm thinking of what something where they slipped into falsifying their records because of an honest oversight then realized they keeping accurate records was tough and so why bother .. etc. -
