I have read that in Canada / USA there can be de facto geographical monopolies or limited choice from the history of telecoms companies plus mergers / acquisitions?
Here in the UK back in 2012 I asked my otherwise decent ISP for any timescales on IPv6 roll out and they were unable to commit; I cited it when migrating away, and have had dual-stack ever since.

I seem to have managed to find the one ISP that has fiber available at my home, has IPv6, does not do IPv4 CG-NAT, and does not have usage caps.
