Barrier for entry was lowered in the 70/80's because there wasn't enough black doctors, standards were lowered again in the 90's because there weren't enough female doctors.

In the last 10-15 years, or so, Indian doctors can pay to shadow a licensed physician for a year in the US and then they're "deemed as passed" US medical school.
