@SnoopJ @regehr one of my aerospace friends once asked me "why do you software people always talk about the Therac-25? In my field, we have so many examples and analyses of safety-critical systems that failed"

My answer was that the Therac-25 was not so much the equivalent of the space shuttle blowing up as it would be the equivalent of the space shuttle blowing up because NASA made it out of papier-mache for no particular reason.
