@silverpill @tadano What pwm said. If there's no mediaproxy enabled, images are simply hotlinked. If it is, Pleroma uses a basic mediaproxy that will forward request to the remote instance, doing it every time unless you set up nginx cache for mediaproxy's endpoint.
