Many of the films in the replies — e.g. Dancer in the Dark, The Lobster — I thought were not worth seeing even the first time: cynicism and/or cruelty dressed up as profundity, trying to make a point about humanity while being indifferent to the humanity of their own characters, who exist only to serve the purpose of a sadistic storyteller. Meh.
Your OP is a real challenge for me! I thought of Audition, but tbh not sure I’m glad I saw it the first time.
