In my career, I've had 1 book returned when the patron died. I waived the overdue fees.
I had another person who died, her parents got her lost bill notice email. They replied that they had donated all of her belongings. I wrote the book off and didn't charge them.

I returned my mom's library books after she passed away. As a teacher she had quite a few out. All fees were waved and I cried with the 2 librarians who were her old friends.

I've returned books borrowed by family members when the chaos has died down after their death. Back when there were fines, they were never issued. A discreet word about the death sufficed
