Haven't had that yet, but would expect that on any protocol or service if it was 404'd for whatever reason. That sounds pretty straightforward and predictable.

I've never had issues with OMEMO outside of larger MUCs and on multiple devices therein. OMEMO in messages between individuals always works reliably. I'd just never use OMEMO in MUCs. Messages are encrypted in transit anyway via TLS, so, whatever. @sendpaws
