downs is caused by an extra copy of chromosome 21.
it happens before conception, when the developing egg cell is supposed to evenly split its 23 pairs of chromosomes. both copies go into one egg cell and the sperm adds a third.
most such chromosome accidents result in a non-viable egg, but an embryo with 3 copies of 21 can develop to full term. it is visible in a newborn baby's face.
