Here it's a mandatory insurance scheme where insurance is then extremely cheap with a small co-payment and health care prices are regulated. I know someone who walked into a hospital feeling unwell and left two weeks later with a quadruple bypass and paid a total of 500 euros.
What you pay extra for is comfort, the basic insurance is in a shared room for example, you can pay extra insurance or out of pocket for a private one, but then the prices are not regulated and sometimes doctors will charge ten times as much.
