Kind of both.
Each business has its own policies (although they're pretty much identical across almost all businesses anyway), but whether they get enforced or not depends on the employees.
For example, I got into Melon Books 1 in Akihabara maskless, and nobody cared.
I got into Melon Books in Chiba, and I was told to wear a mask or they would refuse to sell me goods, but "this time is an exception".
I got into Melon Books in Ikebukuro, and I was forced to leave the store before I could even see half of the store itself.
And when I returned to that same Melon Books in Ikebukuro on another day, all of the sudden nobody cared, and that staffer who kicked me out wasn't there.
