It depends on the visibility of the reply. If it's mentions only then it will just go to you and the accounts mentioned in the post.

If it's a public setting like public or unlisted, it will be visible to your followers and everyone on the servers of your followers, and possibly even further if relay servers are involved.

For privacy purposes, it's best to assume public posts are visible to everyone, because they can be distributed surprisingly far.
