My area (in Kanto, I won't say anything more specific) doesn't care so much, most will wear a face diaper just in case somebody from outside of the town comes and goes full DICKtator mode.
Interestingly, most of Tokyo, Kawasaki, Yokohama, Chiba, and Saitama doesn't care anymore either, but Funabashi, Kawaguchi, Matsudo, Kawagoe, Misato, Kashiwa, Urayasu, and all the other smaller cities within the metropolitan area do, but the smaller towns outside the metropolitan area don't.
