𝓐𝓷𝓭𝔂𝓣𝓲𝓮𝓭𝔂𝓮 𓀤There WON'T BE any rural hospitals. Those who can afford it will pay for "helicopter insurance" to cover the increasingly likely event that they will need to be airlifted ($50,000 and up) to a real hospital which can no longer be found locally.
The rest will die a lot.
