As I understand it, the surgery sites hurt for the rest of your life. (Like FGM in Africa.) His life is a continuous agony of pain in the groin. (And millions in the bank for Big Pharma.) These genital mutilation surgeries are not simple castration (which is relatively free of side effects other than the obvious and low testosterone).
