   Absolutely wildly invasive survey (abuse, alcohol, not just the usual depression check) before my regular checkup with the doc.

   The kicker? The information is NOT shared with my doctor! In other words, this survey was not for my benefit.

   Just a heads up: had I known, I would have declined.
