   Knowing nothing about the context of Big Smoke's order, it's wild to me that a fast food place would have at least 45 combo meals.

     @Paradox Jersey Mike's menu works this way. All of their basic sandwiches are low numbers, and all of the "combination" sandwiches are just number + number. Like say if ham and cheese was #11 and turkey was #13 then ham and turkey and cheese would be #24. Their menu actually goes up to at least 66, if not more. https://www.jerseymikes.com/menu/hot-subs
