VASAviation released a new video in relation to the #aviation accident over the Potomac River—24 hours before you can see a similar incident playing out where the ATC collision alarm was trigger **three times in a row** for a single helicopter.

In safety critical systems, alarm fatigue is a real problem where alarms sound so often that the operator begins to tune them out. The USCSB has talked about this exact issue in a couple of their investigations.
