     I've had White, Hispanic, Asian, and Indian doctors. White or Hispanic are the best. Not because they necessarily know more, but because they tend to listen and will cut corners of necessary.
     
     Asian and Indians are strict rule followers.
     
     I've never had a black doctor.

     It could if other people could identify the patient. If someone knew the patient personally and saw the tweet and put two and two together, that's a violation.
     
     As for medical journals, doctors typically acquire consent.

     The problem here is that the doctor couldn't even claim that the expletive isn't related to the treatment. Saying "NIGGER" is clearly the result of his brain injury. 😏
