@adrianh By "Whatever", do you mean more specialisation in a technical area?
But certainly a common anti-pattern I think, formed from a strange concoction. Juniors want to be promoted for more pay. Seniors often want someone to take over the "difficult" middle management roles. Juniors might not want someone they don't know/trust managing them, etc.
But yeah, we don't value people skills at middle management levels a lot, from what I can see.
