@SleepyCatten In my experience in the UK they basically only offer transdermal, unless there's a particular reason to offer pills.

While it is possible to achieve sufficiently-high levels via transdermal gel or patches, it is typically more-difficult.

& for those on pills, which are typically the cheapest method on prescription, it's just not practical, even if you take the pills sublingually or buccally rather than orally.

As such, if your aim is pure feminisation or masculinisation, you will require a blocker of some kind if you still have hormone-producing gonads.

For most folks, a regular injection (subcutaneous or intramuscular) will give a sufficiently-high, stable level enough to suppress the other primary sex hormone.

Commonly-available estradiol esters are valerate, cypionate & enanthate.

Commonly-available testosterone esters are cypionate, enanthate & undecanoate.

Such injections are ideal for monotherapy HRT (i.e., a sex hormone without medication that blocks the other sex hormone).

A reasonable timeframe should be as soon as possible. Realistically, all a medical professional needs to do is get a baseline blood test to check hormone levels & for signs of issues that may affect HRT.

Assuming the person doesn't want to delay starting HRT for fertility reasons, it's then a matter of discussing HRT options, discussing the effects, & offering the most-suitable meds to achieve their own transition goals.

In an ideal world, all trans folks wanting HRT would be prescribed the safest & most-suitable meds for their needs:

      • within a reasonable timeframe
      • via informed consent
      • via a national healthcare system or otherwise at an affordable cost

... by a qualified professional who fully understands trans HRT.

However, for most folks, these factors will not all occur together, if at all 😞
