Open plastic junction box with an outlet danging from the wires coming out of the box. The two ground wires, upstream and downstream, are twisted together with no ground wire going to the outlet's conspicuous green ground screw. One pair of hot and neutral wires are also swapped, so either this outlet has a hot neutral or it and all the downstream outlets do.

Checking on the previous homeowner's wiring job before plopping all the Ethernet gubbins down nearby... oh. Yeah, that's why I check.
There are at least two things wrong with this wiring job. Sigh.
