I've seen a few times organizations experience a "governance fail loop". When things are fine, governance (like voting/deciding rules) seems unnecessary. Then when things are suddenly NOT fine, it's not only impossible to fix the immediate problem, it's also impossible to add in governance at a time when everybody's suspicious of the governance proposal being used to also make one side win. Anybody else seen instances of this?
