Faction system where you have to carefully navigate the loyalties of the Carcosans, Esoteric Order of Dagon, and something setting-original to keep it fresh, or reject all faith as a memetic defense, etc etc.

Honestly for me the perfect videogame would be if the Bethesda fallout team stopped flirting with lovecraftian themes in sidequests and just dropped us into alt-30s Boston or Arkham or Halifax after an Old One has woken up.
